Summary

Follow-up to merged #1361. That pull request was already merged, so the 200k climb was not in it.

When the first LCD simulate does not finish, raise the query gas from the static envelope by 200,000 until a simulate returns gas_used, then sign ceil(gas_used × 1.2). Three replies with no gas_used stop the climb and sign the static envelope. An included out-of-gas transaction is still not sent again.
